When facilities use H0018
H0018 describes a day of short-term residential treatment in a non-hospital program. The descriptor excludes room and board, which matters: a payer that wants H0018 may pay room and board separately, or not at all.
Many commercial payers do not ask for a HCPCS code on residential lines. Evernorth's billing resource, for example, lists revenue codes 1001 and 1002 for residential with no HCPCS. Where a payer or state program does want one, H0018 (short-term) or H0019 (long-term) is the usual choice.
CMS lists it with coverage code I, not payable by Medicare. Commercial payers and Medicaid programs set their own rules for it.

Common denials and fixes
- Room and board on the same claim when the payer excludes it. CO-97 or CO-96. Check what the contract pays.
- H0018 sent to a payer that does not use it. CO-16 or CO-181 style rejections. Follow the payer's residential billing rule.
- Length of stay. CO-152 or CO-50 at concurrent review. Appeal with day-by-day evidence.

What does short-term mean in H0018?
The CMS descriptor does not set a day limit. H0019, the long-term code, describes stays typically longer than 30 days.
Does H0018 include room and board?
No. The descriptor says without room and board.
