Meaning
0912: Behavioral health treatments and services, extension of 090X: partial hospitalization, less intensive
0912 or 0913
Both 0912 and 0913 are partial hospitalization codes in the 091X range. 0912 is labeled less intensive and 0913 intensive. The difference is usually defined in the payer contract, often by hours per day.
Some payers use only one of the two. Point32Health bills H0035 with 0912, counting half days; Evernorth and BCBS of Mississippi accept either code.

Common denials and fixes
- Tier does not match the contract. Billing 0913 rates for a program the contract defines as 0912, or the reverse. Expect underpayment or CO-150.
- Units counted the wrong way. CO-151 or N362.
- Medical necessity. CO-50. DMHC independent reviewers overturned 87% of PHP and IOP denials that reached them in 2021 to 2026 (an upper bound).
